Community nursing and in-home clinical care bring qualified nursing support to participants with high or complex health needs in their own homes — wound care, continence management, medication management, catheter and stoma care, diabetes management, and clinical assessment and care planning. This enables people to receive clinical support without being in hospital and to live more safely at home.

Registered and enrolled nurses deliver the care and often coordinate a person's health-related supports, developing care plans and training and overseeing support workers who carry out delegated tasks — clinical leadership that is especially important alongside high-intensity personal care. Funding sits within Core or capacity-building supports for nursing that relates to your disability, while acute medical treatment remains the health system's responsibility. When choosing a provider, look for appropriately qualified nurses, strong clinical governance, continuity of care, and the ability to train support workers and coordinate with your other health services.
